Patients can now digitally sign forms! :memo: :writing_hand: :tada:

When setting up your form templates, you’ll have the option to select a “signature” question type:

If you choose that, then the patient will see a box where they can digitally sign their name before submitting the form back to you:

And once that form is stored with the rest of the patient’s details, you’ll be able to see the signature like this:

It will also show up on the PDF of the form:

This should be helpful if you need to collect your patients’ signed consent on certain documents, and retain a record of it!

This is great! However we have been asking for signatures in LETTERS for years- please tell me this can be done for letters too now you’ve done this for forms :pray:t3::pray:t3:

Thank you, this is a big step forward with forms!

Are you guys working on logic too? Having the ability to collect relevant info only would be extremely helpful over having every patient have to wade through questions that may not be relevant (i.e. Do You Have A Surgeon -> if yes then more questions)
