Patients can now digitally sign forms!
When setting up your form templates, you’ll have the option to select a “signature” question type:
If you choose that, then the patient will see a box where they can digitally sign their name before submitting the form back to you:
And once that form is stored with the rest of the patient’s details, you’ll be able to see the signature like this:
It will also show up on the PDF of the form:
This should be helpful if you need to collect your patients’ signed consent on certain documents, and retain a record of it!
