The next time you edit your treatment note templates or patient form templates you’ll now see a new “Date” field type question as well as the ability to add an “Other” option to your list of possible answers for your multiple choice or checkbox questions:
With the new “Date” field type, you’ll be able to select past or future dates quickly. To add this to your templates, head to Settings → Treatment note templates or Settings → Patient form templates. From here, select the template you’d like to edit.
When adding a new question, you’ll now see the option to select “Date” from the “Choose type” drop-down. Once added the new field and you save your changes, when creating a treatment note or a patient fills out their form, there will be drop-downs to select the date:
When you include a multiple choice or checkboxes field type, you’ll now see the option to add an answer or add an “Other” option. If the answer isn’t available in the list, the “Other” option will allow you or your patient to write in a new custom value.
To add this to your templates, head to Settings → Treatment note templates or Settings → Patient form templates. From here, select the template you’d like to edit.
You’ll now need to add a checkbox or multiple choice question to your template:
When adding your answers to your multiple choice or checkbox questions, you’ll now see an option to “Add Other”. Once added, make sure to save your changes.
Now when you or your patient are filling out your treatment note template or patient forms, when they select “Other”, they’ll see a text box appear so they can input their own custom value.
