Client Forms not saving details

Anyone else getting a lot of client forms returned blank? i have had some clients say they filled the form out, but no results come through on my end

Hi @brentonwest. I haven’t had this issue. Is the whole form blank or just certain sections?

We haven’t had this, but we do still get a number of patients that say they have filled in the form and submitted it but we don’t get it. We cannot find a pattern so we don’t really know what has actually happened.
